Introduction and basis
A-Central nervous system is called Neuraxis…
B-It is composed of Brain____spinal cord_____Nerve roots____Peripheral nerve..
C-They act as continuance(one unit) so any tension In CNS (brain and spinal cord) causes tension in nerve roots and peripheral nerves
-In case of disc collapse as compression occur on C4 for example causes it defect in its response to the tension occur in spinal cord…
-So movement in CNS (neuraxis) causes movement of peripheral nervous system and vice versa.
D-If entrapment of nerve occur at any point of body…must predispose entrapment else where in any other point of body.
So should be treated by releasing this entrapment from the nerve.
E-Neves lengthen and shorten according to movement of segment.
F-Peripheral nerves at end of spinal cord are called caudaequina
Tension tests
A-Lower limb
A-Passive neck flexion(PNF)
B-Straight leg raising.(SLR)
C-Prone knee bent(PKB)
D-Slump test(ST)
In each test we should determine five points:-
-Application of test.
-Indications
-Response to test in normal person
-Alternations and variations (sensitization
-Biomechanics
A-Passive neck flexion(PNF):-
-Application :-
__Patient is supine lying to relax all body and neutralize body parts to avoid tension on any peripheral nerve on any other part of body.
___Therapist put one hand under occipit and other hand under neck and make passive flexion of neck till chin reach chest.
___should sustain the position to about 30 seconds to wait for full stretch and wait till compromisation of blood vessel called vasa nervosa causing ischemia to lead to aggrevation of pain
-Passive neck flexion causes tension on neuraxis (spinal cord and membranes surrounding it specially duramater
-Indications:-
-Discogenic problems (disc prolaps or herniation "protrusion",,disc pulge) so press on nerve roots
-Response of normal person to test:-
-PNF must be painless for normal person
-And very rare about less than 20% feel stretch on cervicothoracic junction
-sensitization:-
-Apply more stretch by flexion of upper thoracic region …Why?
If the herniation or disorder is in lower cervical spines for example C7,T1
- But only neck flexion with out association of thoracic region when disorder is in upper cervical spines as C3,C4 or C4,C5
-Biomechanics:-
-Stretch on neuraxis and membranes surrounding it specially duramater
B-Straight leg raising(SLR):-
-Applications:-
-Patient is supine lying for same mentioned reason
-Raise patients leg(Flexed hip and full extended knee)
-Biomechanics:-
-stress (Tension)on sciatic nerve
-Indications:-
-Compression on sciatic nerve by hamstrings
-Discogenic Problems in lower lumbar region "sciatica
-Response in normal:-
-Pain on back of thigh, knee, leg down to foot..
